FamCamp Log book for coupon redemption

Does anyone know how to get a new FamCamp log book. When we stopped RVing a couple of years ago, we had completed tier 3, 30 famcamps. I did receive my coupons but now that we are back traveling, I'm wondering how I go about get my Tier 4 log book. Anyone know? Thanks!

The USAF Frequent Camper Program has been having some problems. This ARTICLE will help explain things. Updates are at the bottom of the article.

If you wish to continue onto tier 4, you'll need to use your same log book as before. Otherwise, you'll start at the beginning if you get a new log book. You might get very lucky and the Program Office "might" have a record of your previous three tiers.
